Nazi Jewish Questionnaire 1942 – Occupied Poland

This is the first page of a questionnaire given in 1942 to residents of formerly Soviet-occupied Poland intended to identify Jewish professionals for deportation and eventual execution. The document to be completed for the German authorities by health professionals reporting for the first time (Fragebogen zur erstmaligen Meldung der Heilberufe) was filled out by Ruben Keineger, a dentist. His photograph has been affixed to the document and the designation "Jude" with two Stars of David stamped at the top.The document measures 9 x 12.
