This is an incredible Personnel Record / W4 Form signed an amazing four times by Hollywood legend Marilyn Monroe as she was about to begin her career towards becoming a successful cinematic actress following years struggling to parley a modeling career into the success of the big screen. Dated December 7, 1949, a young 23 year-old Monroe has signed this historic document, no less than four times, including a rare signature of her legal married name from her earlier first marriage to Jimmy Dougherty, “Norma Jeanne Dougherty.” The document is further graced with two other cursive signatures, signed “Marilyn Monroe,” and a fourth signing by her of her printed “Monroe” signature. As evidence of Monroe’s complicated psyche, she interestingly writes “Not Living” in responding to a question about her father’s and mother’s full name despite not knowing her father’s identity and knowing that her mother was still alive and well. Other details no less interesting is her address, "1301 N. Harper Ave." significant because it was part of the apartment complex where her acting coach Natasha Lytess lived, and where Monroe later returned after the death of her agent and lover Johnny Hyde (at which point she is purported to have attempted suicide). Hyde's employer, the famed William Morris Agency, is listed here as the emergency contact. Also of note, Monroe intentionally provides an incorrect birth year ("28") and age ("21") to pass herself off as two years younger. Finally, for her marital status, she misleadingly checks "Single" rather than "Divorced." Monroe’s off camera sensitivity and insecurity is epitomized in Elton John and Bernie Taupin's beautiful ode to Monroe, "Candle in the Wind," the song begins "Goodbye Norma Jeane..." Marilyn Monroe will always remain, at different times and for different reasons, Norma Jeane. This rare personal biographical record signed four times including in such rare form from one of the most collected celebrities in the history of Hollywood is a treasure to be cherished by even the most advanced collector.
